Instructions:

RETURN THE FOLLOWING

1) Return completed FON application with registration, along with a completed applicable City of Portland permit form to your specific area

2) Include complete FON fees -check made payable to Festival of Nations to the address on the Festival of Nations application.  The City of Portland has graciously waived the permitting/licensing fee for all participants.
